This presentation reviews what transition plans are, what should be included in a transition plan, and why transition plans are an important part of the IEP process once a student reaches transition age. It discusses what options students with an IEP have post-graduation, including, but not limited to, post-secondary education, vocational training and education, and community participation. This presentation will discuss the vital role a speech-language pathologist plays in a transition plan, and it concludes by sharing therapy materials and ideas that are geared toward increasing a student’s functional life skills.
